Why didn't my absence request form submit?

If your form doesn't submit, you will probably get an error message...

Error 500 message:

HTTP Web Server: Lotus Notes Exception - Unable to interpret Time or Date 
You have entered your date(s) of absence in a format that will not submit.  Always, use the exact format shown in the form example.  [i.e. 06/01/2009]


If you get this message:
Please use your browser's "Back" button to complete required fields (*) on form.  Required fields are always noted with a *.  If one is skipped, you will not be able to submit the form.  [Field Number 14 is a frequently missed field that results in this message.]

Remember:  If you are not in the current database, you will not be able to submit a form.

Why do I get "user not authenticated" when I try to access the absence request form?

If you try to access the Absence Request Form and can not gain entry, do the following:

Members, re-type your email username and password in the popup window. (Do NOT put in your whole email address.)

New personnel, if you cannot access the "Absence Request Form" after attempting to use your email username and password, email the webmaster requesting access.  Insert "Absence Request - Access Requested" in the subject line of your email. In the body of your email, give your whole name, position, and base location of employment.

Check the URL to make sure you are in the current database.  [For example, if this is the 2009-2010 school year, part of the URL will read "absence_09.nsf" ]
